We are committed to the success of your treatment and care. This document is to help you understand that payment of your bill is part of your treatment and care. For your convenience, we have answered a variety of commonly asked financial policy questions below. If you need further information about any of these policies, please speak with a Billing Specialist or the Administrator.


Commercial Insurance (indemnity/"regular" insurance)

Payment of the office visit is due at time of service.

Our staff will file the primary insurance claim on your behalf.


HMO & PPO plans with which we have a contract

If the services you receive are covered by the plan: All applicable co-pays are due at the time of the office visit.

If the services you receive are not covered by the plan: Payment in full is due at the time of the visit.

Our staff will file the primary insurance claim on your behalf.


Blue Cross/Blue Shield HMO/PPO

Payment in full for office visits, x-ray, injections, and other charges is due at the time of office visit. Surgical patients will be requested to make a pre-payment of one-half the estimated surgical charge prior to date of surgery. This pre-payment arrangement should be made with the business office.

Our staff will file the primary insurance claim on your behalf. You will be paid directly.


Point of Service Plan or Out of Network PPO

Payment of the office visit is due at time of service.

Our staff will file the primary insurance claim on your behalf.


Medicare

Payment for any services not covered by Medicare is due at the time of service. If you have Regular Medicare as primary, and also have secondary insurance or Medigap: No payment is necessary at the time of the visit.

Our staff will file your primary and secondary claims. Additionally we will bill you for any deductible or co-payment.


Medicare HMO

All applicable co-pays and charges for non-covered services is due at the time of the office visit.

Our staff will file your primary and secondary claims.


Worker's Compensation

If we have written verification of the claim with your carrier: No payment is necessary at the time of the visit.

If we are not able to verify your claim: Payment in full is due at the time of visit.

Please provide, and bring with you to your appointment, all verification information.

Our staff will call your employer or insurance carrier prior to appointment to verify coverage for your visit. Additionally, we will file your claim.


Caterpillar - Hourly Staff

Payment of the office visit charge is due at time of service. Payment of any services that are not covered by your insurance is also due at time of service.

Our staff will file the primary insurance claim on your behalf.


Caterpillar - Management

Payment of 20% of the office charges is due at time of service. Payment of any services that are not covered by your insurance is due at time of service.

Our staff will file the primary insurance claim on your behalf.


Caterpillar - Worker's Compensation

If we have the Caterpillar Work Comp authorization verifying the claim: No payment is necessary at the time of the visit.

If authorization is not provided: We will process the claim under your group medical coverage and collect payment from you under that plan.

Our staff will file your claim.


Medicaid

You are responsible for obtaining a referral from your primary care physician prior to the visit. Payment of your co-pay is due at time of service.

You are responsible for bringing your IPA card for each visit.

Our staff will file your primary insurance claims.


No Insurance

Payment in full is due at time of visit. Surgical patients will be requested to make a prepayment of one-half the estimated surgical charge prior to date of surgery. This prepayment arrangement should be made with the business office.

Our staff will work with you to settle your account.
